Dr. Pooja A. Patel is an occupational therapist and gerontology specialist with deep experience working alongside older adults, their families and the health-care system. She founded Aging Together, LLC to help families navigate aging-related transitions including dementia care, fall prevention and long-term-care planning.

Her mission is to empower unpaid family caregivers through education, resources, peer support and proactive care planning so that aging, healthcare decisions and caregiving journeys become more manageable and less stressful.
